New Patients
Welcome to Chambers Foot & Ankle
Welcome to Chambers Foot & Ankle. We understand that visiting a new podiatrist can feel stressful, especially if it is your first visit. Our team is here to make the process as easy as possible. From scheduling your appointment to meeting with Dr. Chambers, we are happy to answer your questions and help you feel prepared for your visit.
During your first appointment, Dr. Chambers will discuss your symptoms, review your medical history, and learn about your treatment goals. He will perform a thorough evaluation to find the cause of your foot or ankle problem and create a treatment plan based on your needs. Whenever possible, treatment starts with conservative options. Advanced therapies or surgery may be recommended if they are the right choice for your condition.
Preparing for Your Appointment
To help make your visit easier, please complete your new patient forms before your appointment if you can. Bringing your completed forms can save time and give you more time to talk with Dr. Chambers. If you can’t complete them beforehand, our team will be happy to help you when you arrive.
Please bring the following to your appointment:
A photo ID and your insurance card.
A list of your current medications.
Any medical records or imaging related to your foot or ankle condition.
Shoes or orthotics you wear regularly, if they are related to your symptoms.
